Response - 30 minutes.

Hours 3 PM - 7 AM.

Back up call every night.

Case Mix: Gen, Endo, OB (less than 450 deliveries), Ortho (some spines), URO, GYN, Podiatry, cardiac (less than 380/year- cath lab and TEE)Must be Proficient: Spinals, blocks, Epidurals# of Providers per day: 4# of OR's: 4

CV room: 1

Active License

Credentialing Time Frame: 45-90 days

Case Logs Required: proof of epidurals, spinals and blocks
